Use the form below to request a copy of the desired report. Requests are typically processed within 5–7 business days. You will be contacted by district staff once your request has been reviewed. Medical report requests not originating from the patient must be accompanied by a HIPAA compliant authorization for release of information signed by the patient or legal representative.

Insurance Company Requests

For all insurance company requests — list the company name, contact, and claim number in the Additional Information section above. Requests by insurance companies may require an Arson Immunity Letter as per RSMo 320.081.

Please note: Complete Origin & Cause Fire Investigation Reports are not public record.

West Peculiar Fire Protection District may charge a fee for records and staff time pursuant to the Missouri Sunshine Law (RSMo Section 610.026). Residents of the district are normally not charged for copies of reports released directly to the resident.
